Background: Infantile colic remains a common health problem, with a global prevalence of 30–60%, approximately 40% in Indonesia, and 69.4% among infants under one year of age receiving complementary feeding in North Sumatra. Infants with a history of formula feeding are also at greater risk of developing defecation pattern disorders. One non-pharmacological intervention that may help overcome these conditions is the I Love You (ILU) massage technique. Objective: To determine the effectiveness of the I Love You (ILU) massage technique in reducing colic and improving defecation patterns in infants with a history of formula feeding. Methods: This study employed a quasi-experimental design with intervention and control groups without randomization. A total of 70 infants were selected using purposive sampling, consisting of 35 infants in the intervention group and 35 infants in the control group. Data were collected through observation and assessment of colic conditions and defecation patterns before and after the intervention. Data were analyzed using the Mann–Whitney test with a significance level of 0.05. Results: The findings showed that the I Love You (ILU) massage was effective in reducing colic in infants, with a p-value of 0.000 (<0.05), a mean rank of 24.81, and a sum of ranks of 868.50. The I Love You (ILU) massage was also effective in improving defecation patterns, with a p-value of 0.000 (<0.05), a mean rank of 44.53, and a sum of ranks of 1558.50. After the intervention, most infants in the treatment group no longer experienced colic and demonstrated normal defecation patterns. Conclusion: The I Love You (ILU) massage technique is effective in reducing colic and improving defecation patterns in infants with a history of formula feeding.
